Lisa Holtsclaw DO

Dr. Holtsclaw is Board Certified in Family Practice medicine and a lifelong resident of Fort Wayne. She graduated from Kirksville College of Osteopathic Medicine in 1993 and completed her residency training at the Fort Wayne Medical Education Program in 1996. Dr. Holtsclaw was employed by Parkview Medical Group prior to opening her own practice. It is her desire to effectively manage a patient centered medical practice allowing for increased patient interaction and input.

Dr. Holtsclaw is married and has three children, 2 boys and 1 girl. She enjoys spending family time at home in the pool or tubing at their lake cottage. She also relaxes by reading or going to movies.

Sarah Heck

Sarah Heck is a Board Certified Family Nurse Practitioner. Sarah shares the same high standards of patient care as Dr. Holtsclaw. Sarah graduated from the Nurse Practitioner program at Indiana Weslyan University, Indianapolis, in 2005 and has been with Holtsclaw Medical Centre since 2006.

Sarah is married with 3 young boys. She enjoys spending time with her family, keeping busy with various activities, and fixing up their new house.

Jeff Holtsclaw

Jeff Holtsclaw is Practice Administrator at HMC. He earned his B.S. in Business Administration from Indiana State University in Terre Haute, IN. Jeff is a native of South Bend, IN and was the President of Wireless All-Stars a communications company in Northern Indiana for six years prior to transitioning full-time to the practice.
